Transparent Classroom
The guides use Transparent Classroom to track students’ work and progress. You will have a parent login to access information about your child’s experiences at school. Transparent Classroom can also be set up to text or email you photos and updates about your child on a daily or weekly basis.

Join the fun by becoming an active member of our St. Philomena Montessori PTO! SPM's Parent Teacher Organization is open to any current or former parents. Our PTO's mission is to support the needs of the school and its families and to educate the community on the Montessori educational approach. We sponsor community and school events such as the family Sock Hop and Big Screen Movie Night.

Enrollment FormList Item 1
An enrollment form for Toddler & Preschool is required with enrollment at SPM.

Medical StatementList Item 2
All students are required to have a completed Medical Statement on file before their first day of school. This has to be updated annually.

Pick Up AuthorizationList Item 3
A pickup authorization form is required to have on file for each family. This form is used for Toddler, Preschool, Kindergarten and Lower Elementary.

Return from AbsenceList Item 4
This is required upon your child's return from absence for any reason.

Request for Administration of Medication
This form is to be completed for each prescription or non-prescription medication that a child needs to receive while in care. It is NOT required to be completed for topical productions, lotions, or if the medication is required by a health care plan (JFS01236) as in next item.

Child Medical/Physical Care Plan
This form is to be completed for each prescription or non-prescription medication that a child needs to receive while
in care.
It is not required to be completed for topical products, lotions, or if the medication is required by a health care plan (JFS 01236).
This form is required for epipens and inhalers.

ODH Epinephrine Form
This is an additional form which is required prior to a child possessing an epi at school.

AOC Asthma Inhaler Form
In addition to the child's medical/physical care plan, an AOC Asthma Inhaler form must also be completed for students possessing an inhaler.

This form is required to be filled out by a parent requesting a non-presciption medication (including topical creams and lotions) to be administered at school.
